Notice of Privacy Practices (HIPAA)
As a supplier of durable medical equipment, Star DME Inc. follows the Health Insurance Portability and Accountability Act (HIPAA). Our Notice of Privacy Practices explains how we may use and share your health information, and your rights, including your right to get a copy of your records and to ask us to correct them.
A paper copy is available in our store at 141-04 Rockaway Blvd, Suite 1F, Jamaica, NY 11436, or call (917) 300-2692 and we will mail or email one to you.
What our website collects
When you use the Check coverage form, we ask for:
- your name, phone number and, if you choose, your email address
- whether you have insurance and which plan
- which product you are asking about
- how you found us, for example Google or a link from your doctor, and whether you used a phone or a computer
We use this only to check your coverage, contact you about your request and keep records required for insurance billing. We do not sell your information and we do not use it for advertising.
How we store and protect it
Form answers are stored in a private database on our web server, sent over an encrypted (HTTPS) connection, and can only be seen by Star DME staff with a password. When a new request comes in, staff get a text or app alert that contains only a reference number, never your name or health details.
Visitor statistics
We count visits to our website to see which pages are useful and how people find us. This count uses an anonymous ID that changes every day. We do not store IP addresses, we do not set tracking cookies on visitors' devices, and we do not use Google Analytics or advertising trackers.
Other services on our pages
Our pages load fonts from Google Fonts, and videos on our website play through YouTube in privacy-enhanced mode. These services may see your device's IP address when they load. Links to Google, Facebook, Yelp and other sites follow those sites' own privacy policies.
Your choices
You can ask us what information we have from the website form, ask us to correct it, or ask us to delete it when we are not required to keep it for insurance or legal reasons. Call (917) 300-2692 or email orders@stardme.com.
Please do not email medical records. Have your doctor fax them to (844) 438-5893.
Changes
If we change this policy, we will update the date at the top of this page.
