Diabetic shoes
Diabetic shoes and inserts
Extra-depth shoes and inserts that protect feet affected by diabetes, fitted in our Jamaica, Queens store and billed to your insurance.
What we supply
Extra-depth shoes (A5500)
Roomy, cushioned shoes with extra depth to fit inserts and reduce pressure and rubbing on sensitive feet.
Heat-molded inserts (A5512)
Multi-density inserts shaped to your foot to spread pressure away from calluses and sore spots.
Custom inserts (A5513)
Inserts made from a model of your foot when you need more support than a heat-molded insert gives.
Orthopedic shoes
Supportive shoes for other foot conditions. Coverage depends on your plan, so call and we will check.
How insurance covers diabetic shoes
Medicare Part B
Each calendar year: one pair of extra-depth shoes plus 3 pairs of inserts, or one pair of custom-molded shoes plus 2 extra pairs of inserts. You pay 20% after the Part B deductible.
Who qualifies
You have diabetes and at least one foot problem: past amputation, past foot ulcer, a callus that could become an ulcer, nerve damage with calluses, foot deformity, or poor circulation.
NY Medicaid and other plans
Medicaid and many commercial plans may also cover diabetic footwear when it is medically necessary. We check your plan before you pay anything.
What we need from your doctors
Medicare needs two things: a statement from the doctor who treats your diabetes (MD or DO) certifying you need the shoes, and an order from your podiatrist or other qualified doctor after a recent foot exam. Both can be faxed to (844) 438-5893.

Common questions
Does Medicare cover diabetic shoes?
Yes, if you have diabetes and a qualifying foot problem, Medicare Part B covers one pair of extra-depth shoes and 3 pairs of inserts each calendar year (or custom-molded shoes plus 2 extra pairs of inserts). The doctor treating your diabetes must certify that you need them, and a podiatrist or other qualified doctor must order them.
Can I get new diabetic shoes every year?
Yes. Medicare coverage resets each calendar year, so if you still qualify you can get a new pair of shoes and new inserts each year. Your doctor needs to send updated paperwork.
What is the difference between A5500, A5512 and A5513?
A5500 is the billing code for extra-depth diabetic shoes. A5512 is a heat-molded multi-density insert, and A5513 is a custom insert made from a model of your foot.
Do I have to come to the store?
Yes. Diabetic shoes and inserts need to be measured and fitted in person so they protect your feet and do not cause new sore spots.
